Regulation of the Minister of Health on the model order for the supply of medical devices and the model repair order

Please note that as of 1 July 2014, it is necessary to use new order templates for the supply of medical devices and models of the repair order. In accordance with the statutory delegation contained in the Law of 12 May 2011 on the refund of medicines, foodstuffs for particular nutritional uses and medical devices (OJ C 133, 11.12.2011, p. 1). U. No. 122, item. 696, as amended. d.), a regulation of the Minister of Health of 6 December 2013 on the model order for the supply of medical devices and a model repair order (Journal of Laws) was issued. 30 December 2013), which contains the designs in question.

Part A of the model procurement order form consists of the part filled in by the person authorized to issue the order or continue the supply, the part filled in by the employee of the regional branch of the NFZ (confirming the entitlement to supply the requested medical device) and the part filled in by the entity carrying out the activities of supplying medical devices (confirming the receipt of the device from the healthcare provider).

Part B of the form, on the other hand, refers to entities with rights to supply products on a monthly basis, and must include, inter alia, detailed confirmation of the orders issued and their execution.

On the other hand, the medical device repair order form consists of the part filled in by an employee of the regional branch of the NFZ (and information for the recipient confirming that, in the case of repair and the use of part or all of the limit, the useful life of the medical device is extended in proportion to the used part of the repair limit, rounded down to the full month), as well as the part confirming the repair of the device at the healthcare provider and the receipt of that device.

It should be noted that, according to the above-above regulation, only the forms of model orders for the supply of medical devices and model orders for the repair of devices have changed, while the procedure for sourcing medical devices, which requires confirmation of orders in the provincial branch of the National Health Fund, remained unchanged.
